Where can I find a good printable stress test to use in a class?

I teach a group of mentally ill adults at a treatment center. Tomorrows topic is stress management. Can anyone tell me where I can find a good, simple stress assessment?

Do you have to administer it or are they capable of answering simple quiz questions? I googled it and found a helpful link with many quizzes.

    Try downloading meditation audio files. The best technique I know is to listen to them while focusing on the way your breath is. The way it goes in and out, and not paying attention to any other thought that crosses your mind. Meditation and breathing go together and relieve stress quickly.
